An Australian Defence Force (ADF) member submitted a falsified service document, known as a Basic Fitness Assessment, in order to maintain their basic fitness requirement for ongoing service.

The matter was reported after irregularities were detected when the documents were processed in the Personnel and Pay Information System.

Further enquiries established that the assessor listed in the records did not conduct the Basic Fitness Assessments or fill out the relevant forms.

The ADF member was found guilty of offences under the Defence Force Discipline Act 1982 and received a 12-month formal warning.
