The Department of Defence investigated allegations of the theft of a significant number of military store items. The investigation found no evidence of criminal activity but identified significant systemic issues within the asset management of military stores. These included staff not following procedures or policy and a lack of staff oversight by supervisors. This combined with environmental factors, including the users relocating to different bases, contributed to the conduct.

The case highlights the importance of initial fact finding activity and physical audits rather than sole reliance on digital records relating to stores and inventory. This matter may have been resolved prior to the referral for investigation if a more thorough compliance approach had been adopted when the suspected losses were first detected.
